At your first appointment, our team will:
Go over your dental and medical history forms
Conduct a thorough examination of your teeth and gums
Take digital X-rays of your mouth
Examine your teeth for cavities or early decay
Clean and polish your teeth for a fresh, healthy smile
Develop a customized treatment plan tailored to your needs and answer all your questions
Adult Patient Forms
Please complete the new patient forms before your first appointment, and don’t forget to bring your insurance card if you have one.
